Chinle Vet Center

This MVC has openings for a Readjustment Counseling Tech (who will be the driver), and a Readjustment Counselor. The counselor needs to be a licensed LISW or LMSW, and the driver preferably a veteran of the War on Terrorism. Case Manager

Home Care Assistance is hiring a full time Case Manager to work with its elderly private pay client population. The Case Manager must have at least 3 years' experience in the home health field. Excellent communication skills and computer literacy are also required.

The position requires rotating "on call" supervision for caregivers and new clients which may require emergency PRN shift fulfillment and scheduling using software. The Case Manager must be able to handle/resolve issues daily and during any on call, after hour rotation emergencies in a professional manner. Field home visits are required.

Minimum requirements:

- 3+ years experience in home health
- MS Office skills a MUST
- Must pass a local and national background check
- No driving convictions including DWI
- Must meet standards of professional behavior and appearance
- Degree in Human Services preferred but will accept equivalent experience

School-based Health Center Behavioral Health Quality Improvement Trainer

Envision New Mexico is a healthcare quality improvement program of the Department of Pediatrics at the University of New Mexico Health Sciences Center. We work with children's health practitioners to make sustainable changes in practices and communities to improve the quality of health care for children and families in New Mexico. This position will help us in our work with School-based Health Centers around the state, helping the primary care and behavioral health providers adopt best practices in screening, assessing and treating students with common behavioral health issues. This position will work as part of a team, travelling to SBHCs around the state and implementing Model for Improvement activities, providing training and technical assistance on a variety of topics. We anticipate the work will require about 20 hours/week, hourly rate negotiable.
