Primary responsibilities of the Care Manager Social Worker include:
o Visiting patients in their homes, assisted living facilities and other community-based settings
o Delivering a holistic approach to coordinated care based on member needs
o Identifying early risk factors and conducting ongoing assessments
o Creating care plans and focusing on disease management
o Functioning as an advocate for members and decreasing unnecessary hospitalizations
o Collaborating with the member’s Physician to deliver and coordinate necessary services
o Building relationships with members and their families; assisting them with proper health choices
o Ensuring cases are documented in a timely manner
o Participating in team meetings
The requirements for this role are as follows:
o A Bachelor's level or Master's level Social Worker
o Clinical experience
o A background in long-term care (could include: home health, hospice, public health, assisted living and community-based nursing)
o Computer proficiency
o Within the service area for DSL or Cable
Additional assets include:
o An LSW or LCSW with a current and unrestricted license in their state of residence
o Case management experience
o Bilingual Spanish or Native American and English speaking skills
o Medicare/Medicaid experience
o A background working in pediatrics special needs
o A background in behavioral health
o Experience with electronic charting

School Psychologist

Licensed Clinical Social Worker
Align Staffing is seeking a Licensed Clinical Social Worker to provide counseling services to a military community as the primary source of care for clients seeking personal counseling or who are involved in alleged/substantiated child/abuse cases. The position is located at Cannon Air Force Base, New Mexico.

Provide case management, counseling, therapy, educational programs, command consultation and some social work services.

Master's or Doctorate Degree from a Council on Social Work Education (CSWE) accredited school. Must possess and maintain a social worker license from a U.S. jurisdiction, allowing the social worker to work independently without clinical supervision.

Minimum 2 (of last 3) years full-time post Master's Degree experience in group work as a group facilitator or co-leader, and/or in professional clinical counseling service programs to men/women/children who are abusive/abused.

Hourly rate up to $25 per hour, depending on experience.

Knowledgeable in family violence treatment and child sexual abuse assessments.
