Hospice Social Worker

Will be an integral part of the Hospice Interdisciplinary Team and as such is responsible for assessing the psychosocial needs of the hospice patient and family.  Understanding that psychosocial services must be delivered according to accepted standards of practice, the social worker, guided by the patient/family’s needs and wants, coordinates the resources of the family, the hospice and the community into an integrated program of support. Some work outside of normal business hours is required, intense involvement with patients  and their support systems, work mostly in patient homes, must have knowledge and sound understanding of hospice philosophy and concepts of care. Bachelor’s Degree in social work from a social work accredited by the Commission on Accreditation of the Council on Social Work Education, State Certified as a Social Worker. Email resume to
